Abstract
Introduction: Diabetic foot ulcer (DFU) is one of the complications of diabetes
mellitus that often leads to high morbidity and an increased risk of amputation. The
healing process of DFU is influenced by various factors, including nutritional
status, blood glucose control, and therapeutic interventions. Moringa oleifera leaf
extract is known to possess antimicrobial, anti -inflammatory, and antioxidant
properties that have the potential to accelerate wound healing; however, scientific
evidence regarding its application for grade II DFU remains limited. This study
aimed to analyze the effectiveness of Moringa oleifera leaf extract ointment on the
healing process of grade II DFU using a mixed-method concurrent triangulation
approach.
Methods:This study integrated quantitative and qualitative analyses
simultaneously to obtain a more comprehensive understanding. The population
consisted of 30 DFU patients, with a sample of 26 pati ents who met the inclusion
criteria: (1) diagnosed with grade II DFU based on the Wagner classification, (2)
willing to participate in the study, (3) blood glucose level below 300 mg/dL, and (4)
no history of allergy to the ointment ingredients. The quantitative approach
involved measuring the Bates-Jensen Wound Assessment Tool (BWAT) scores on
days 0, 3, 6, 9, and 12, and analyzing the data using the Repeated Measures ANOVA
test.
Results:The findings revealed a significant reduction in the mean BWAT scores
after the administration of Moringa oleifera leaf extract ointment, with statistically
significant differences observed at each measurement point (p < 0.05). The
consistent decrease in BWAT scores indicated improved wound conditions in the
majority of participants. Further analysis confirmed that the intervention’s
effectiveness remained significant after Greenhouse-Geisser correction for
sphericity. Qualitative findings indicated that most patients felt comfortable, found
the ointment easy to use, had a positive perception of herbal therapy, and
demonstrated high adherence to wound care, although some experienced
challenges such as boredom or time constraints.
Discussion:The integration of both quantitative and qualitative findings
strengthens the conclusion that Moringa oleifera leaf extract ointment is not only
clinically effective but also well-accepted and psychosocially supported by grade II
DFU patients.
